What Is a Blank Scope and Delegation Matrix?

A blank scope and delegation matrix is a table with the practice's tasks down one side and its roles across the top, where each cell is filled in by the practice from its own verified sources and signed by the accountable people. It is a decision record, not an answer key. The matrix ships empty because who may perform what is a question for the practice, its prescriber, and the relevant authorities.

Why blank

Scope and delegation rules vary by jurisdiction, role, setting, and agreement, and they change. Any pre-filled matrix risks being wrong for the practice using it. A blank matrix forces each cell to be answered from a source the practice can point to, and it makes unanswered cells visible.

How cells get filled

Each filled cell should reference a verification trail entry. A cell with no reference is an assumption. Where the practice has not verified an answer, the cell stays blank or is marked "not verified," and the question goes to the open questions register. Nobody should be assigned a task based on an empty or unverified cell.

Signing and reviewing

Once completed, the matrix is reviewed and signed according to the practice's own requirements, and it carries a revision number and review date like any other controlled document. Any change in staff, services, or verification answers is a trigger for review.
