When your medical director changes
Orders and protocols carry the authority of the prescriber who signed them. When that person leaves, the practice needs a written handoff and a new arrangement before those documents are used again.
What has to exist
- An exit record for the departing prescriber: what was reviewed, what stops, the effective date.
- A list of every document the departing prescriber signed.
- A new written arrangement: scope, delegation, coverage, chart review, suspension authority.
- Re-signature of each clinical document by the incoming prescriber.
- A verification entry for any services that change status in the gap.
