What Belongs in a Clinic Launch Readiness File?

A launch readiness file holds the evidence that each opening workstream reached a known state before the first patient: the decision records, the verification trail, the completed and signed documents, the open items with owners, and a final go or no-go record. It is not a to-do list. It is the file you would hand someone who asked how you know you were ready.

Why a separate file

During opening, documents are scattered across email, shared drives, and binders. On launch day, nobody can say with confidence what was finished. A readiness file pulls the proof into one place and forces the question of whether each workstream is actually done or just believed to be done.

How to organize it

Organize by workstream rather than by date. Typical sections are space and facility, services and menu, clinical oversight documents, forms and charting, software and records, staffing and orientation, emergency preparedness, and vendors. Each section starts with a one-line status and the name of the person who confirmed it.

Anything not finished moves to a short list of open items at the front, each with an owner and a date. An open item is acceptable. An open item that nobody owns is not.

What to include

• Workstream index with status and confirming owner
• Decision records for major choices
• Verification trail entries, dated and sourced
• List of signed practice documents, with signature dates
• Open items list with owners and due dates

• Go or no-go record for opening day
